A Path of Care Home Health is seeking a skilled and compassionate Field Nurse (LPN) to join our team. As a Field Nurse, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

About the Job

This is a PRN (as-needed) position, offering a flexible schedule and the opportunity to work with a dynamic team. As a Field Nurse, you will be responsible for visiting patients in their homes, providing hands-on nursing skills, and teaching patients self-care techniques.

Responsibilities
  • Provide high-quality nursing care to patients in their homes
  • Administer injections and wound care as needed
  • Teach patients self-care techniques and provide education on disease management
  • Communicate effectively with RNs, case managers, and other healthcare professionals
  • Document patient information and maintain accurate records
Requirements
  • Active nursing license in the state of Oklahoma
  • CPR certification or the ability to obtain one within 60 days of being hired
  • 1+ years of nursing experience
  • Valid driver's license and reliable transportation
  • Ability to travel long distances and adapt to schedule changes
  • Basic knowledge about computers
