Standard operating hours are from 8:00 am to 4:30 pm, Monday through Friday. Please be aware that work hours may vary based on participant and business needs, so flexibility is essential.
Hourly Wage:
$19.00 per hour
Job Overview:
At LIFE-NWPA, our Participant Intake Specialists are vital in assessing and coordinating new participants for our programs, overseeing all intake processes.
Key Responsibilities:
- Assess potential program participants to identify their needs and eligibility for enrollment.
- Conduct assessments in the homes of prospective participants.
- Coordinate services with State Health and Human Services personnel.
- Engage in team meetings and training sessions as required.
- Collaborate with the Interdisciplinary Team regarding initial assessments.
- Assist in marketing initiatives alongside the Marketing Manager, as needed.
- Implement the 'steps in the intake' process to facilitate participant enrollment.
- Gather and complete necessary documentation related to participant assessments.
- Maintain required statistical data.
- Support Quality Improvement initiatives.
- Perform other duties as assigned.
- Ability to interact tactfully and patiently with personnel, patients, visitors, and the public.
- Strong organizational skills and ability to follow through on assigned tasks.
- Excellent verbal and written communication skills, with the ability to follow both written and oral instructions.
- Possession of a valid driver's license and reliable transportation.
- Cognitive ability to process and perform tasks ensuring participant safety.
- Effective teamwork skills, along with strong communication, interpersonal, and conflict-resolution abilities. Experience working with adult geriatric patients and their families is preferred.
- High school diploma or equivalent.
- One year of experience working with the elderly or willingness to undergo extensive training as necessary.
- CPR and First Aid Certification (Training Provided).
- Familiarity with Medicare/Medicaid regulations is advantageous.
- Valid State driver's license with a clean driving record.
- Successful completion of background checks, physical examinations, drug screenings, and TB screenings.
- Ability to move intermittently throughout the workday.
- Engage in sitting, standing, bending, lifting, and moving as needed during working hours.
- Subject to frequent interruptions.
- Capable of lifting, carrying, and supporting participants.
This position requires full access to participant, program, and staff records, necessitating discretion to protect individuals and the program.
