Schedule:

Standard operating hours are from 8:00 am to 4:30 pm, Monday through Friday. Please be aware that work hours may vary based on participant and business needs, so flexibility is essential.

Hourly Wage:

$19.00 per hour

Job Overview:

At LIFE-NWPA, our Participant Intake Specialists are vital in assessing and coordinating new participants for our programs, overseeing all intake processes.

Key Responsibilities:
  • Assess potential program participants to identify their needs and eligibility for enrollment.
  • Conduct assessments in the homes of prospective participants.
  • Coordinate services with State Health and Human Services personnel.
  • Engage in team meetings and training sessions as required.
  • Collaborate with the Interdisciplinary Team regarding initial assessments.
  • Assist in marketing initiatives alongside the Marketing Manager, as needed.
  • Implement the 'steps in the intake' process to facilitate participant enrollment.
  • Gather and complete necessary documentation related to participant assessments.
  • Maintain required statistical data.
  • Support Quality Improvement initiatives.
  • Perform other duties as assigned.
Required Skills and Abilities:
  • Ability to interact tactfully and patiently with personnel, patients, visitors, and the public.
  • Strong organizational skills and ability to follow through on assigned tasks.
  • Excellent verbal and written communication skills, with the ability to follow both written and oral instructions.
  • Possession of a valid driver's license and reliable transportation.
  • Cognitive ability to process and perform tasks ensuring participant safety.
  • Effective teamwork skills, along with strong communication, interpersonal, and conflict-resolution abilities. Experience working with adult geriatric patients and their families is preferred.
Education and Experience:
  • High school diploma or equivalent.
  • One year of experience working with the elderly or willingness to undergo extensive training as necessary.
  • CPR and First Aid Certification (Training Provided).
  • Familiarity with Medicare/Medicaid regulations is advantageous.
Requirements:
  • Valid State driver's license with a clean driving record.
  • Successful completion of background checks, physical examinations, drug screenings, and TB screenings.
Physical Requirements:
  • Ability to move intermittently throughout the workday.
  • Engage in sitting, standing, bending, lifting, and moving as needed during working hours.
  • Subject to frequent interruptions.
  • Capable of lifting, carrying, and supporting participants.
