About the Role

We are seeking a skilled Social Worker to join our team in the Adult Intensive Outpatient Program (IOP) at PrairieCare. As a key member of our treatment team, you will provide essential services to individuals with psychiatric needs, promoting their recovery and well-being.

Responsibilities
  • Facilitate admissions, psychoeducation groups, treatment planning, and discharge planning for patients in the IOP service line.
  • Collaborate with psychiatric medical providers and therapists to advance the plan of care and ensure seamless communication with patients, their support system, and community providers.
  • Develop and revise treatment plans, including complex discharge plans, and communicate these with patients, their support system, and the treatment team.
  • Provide psychoeducation to patients, their support system, and the treatment team on various topics, including admission process, diagnosis, symptom management, recommendations, discharge plans, safety plans, patient rights, and financial impact issues.
Qualifications
  • Bachelor's Degree in Social Work required; Master's Degree in Social Work preferred.
  • Licensure in the State of Minnesota required.
  • Experience in a mental health setting preferred.
  • Proficient in Microsoft Office Products, including Word and Outlook.
