Job Summary

The Licensed Practical Nurse (LPN) is a vital member of our healthcare team at Finger Lakes Community Health. As a Patient Care Specialist, you will play a key role in providing high-quality patient care and ensuring the smooth operation of our medical clinical area.

Main Responsibilities
  • Clinical Floor Operations
    • Prepare the clinical area for opening and closing, ensuring a clean and organized environment for patients and staff.
    • Redirect nursing team tasks to optimize patient flow and efficiency.
    • Clean and sterilize instruments, equipment, and clinical areas according to infection control procedures.
    • Restock rooms with medical supplies and assess inventory to ensure timely ordering.
    • Perform Quality Control measures, including monthly control testing and equipment inspections.
    • Attend morning huddle meetings with the patient care team to address patient concerns and review referral progress.
    • Follow up on patient information during pre-visit planning to ensure appointments are scheduled correctly and necessary information is received.
  • Clinical Services
    • Obtain patient vitals and history during the rooming process and notify the nurse or provider of any abnormal findings.
    • Assist with minor procedures and exams as outlined in the NYS clinical scope of practice.
    • Perform patient screenings as outlined in the NYS clinical scope of practice.
    • Complete phlebotomy and lab collection and handling as outlined in the NYS clinical scope of practice.
    • Confirm patient charts have written orders for medications and/or injections from the provider.
    • Administer injections and immunizations as directed by the ordering provider.
    • Complete medication reconciliation and review and send medication refill requests to the patient's provider.
    • Inform patients or designated persons of physician orders, changes in care plan, test results, etc. as outlined in the NYS clinical scope of practice.
  • Medical Record Maintenance
    • Scan clinical documents from patients or clinical staff into the patient chart.
    • Chart services in the patient record, including referrals, medications, vaccinations, injections, screenings, lab orders/results, patient history, and vitals.
    • Chart and lock notes in a timely manner in accordance with Finger Lakes Community Health policies and procedures.
    • Address telephone encounters, actions, and referrals in the electronic health record system in accordance with Finger Lakes Community Health policies and procedures.
Education and Qualifications
  • High School Diploma or equivalent.
  • A current unrestricted NYS LPN license and CPR certification are required.
  • Prior experience is a plus, but not required.
  • The ability to work flexible hours, including some evenings, is required.
  • The ability to communicate fluently in Spanish and English is preferred, but not required.
  • A valid New York State driver's license in compliance with FLCH drivers' policy is required.
Physical Requirements
  • Must be able to hear and communicate with clients and staff via phone, web media, and in person.
  • Must be able to lift up to 40 lbs.
  • Must be able to sit for extended periods of time.
  • Must be able to kneel, crawl, or bend into awkward spaces.
  • OSHA Level 1 certification is required.
