Nurse Manager Position
We are seeking an experienced RN to join our team as a Nurse Manager in our Transitional Care Unit (TCU). In this role, you will be responsible for meeting with hospital personnel and other healthcare facilities to promote Valley View Village as an option for healthcare. Our goal is to provide a loving community atmosphere where our residents can experience independence and dignity.
This is a Full-Time salaried position requiring on-call rotation.
Hours: 8-4:30pm
As a Nurse Manager, you will have the opportunity to work with an amazing team with decades of experience and make connections with our residents. You will be empowered to foster fullness of life for those we serve.
Responsibilities:
- Completes clinical assessments on prospective residents and day one admission nursing observation assessments.
- Completes required admission paperwork with new residents and family.
- Supervises work performed by staff in TCU.
- Attend and participate in IDT process.
- Assist residents with adjustment to their new surroundings.
- Cultivate relationships with discharge planners across different health systems.
- Track admissions data with follow up.
- Assist with discharge process.
- Ensure all admission process and checklist items are completed.
- Perform additional tasks as requested.
Requirements:
- Must have current RN license.
- 1+ year experience in skilled nursing required.
- Previous supervisory experience a plus.
- Knowledge of local, state and federal guidelines related to long-term care and resident rights.
- Ability to learn multiple databases.
- Excellent customer service/people skills.
- Team player and self-starter.
- Strong verbal communication skills
