The University of Kansas Health System is seeking a skilled Clinical Nurse Coordinator to join our Ambulatory team on a PRN basis. As a key member of our healthcare team, you will provide exceptional patient care and support to our patients and families.
Key Responsibilities- Manage patient flow, triage, and direct patient care to ensure seamless delivery of services.
- Act as a liaison between front desk staff, nursing staff, physicians, and clinic manager to facilitate effective communication and coordination.
- Develop and implement patient care plans, defining expected outcomes and resource utilization, in collaboration with providers.
- Provide nursing support and coordination to optimize patient outcomes and improve quality of life through clinical and educational programs.
- Support patient self-management of disease and coordinate continuity of care with patients and families.
- Manage high-risk patient care, including patients with multiple comorbidities or high risk for readmission.
- Develop relationships with patients and utilize assessment skills to identify physical, psychosocial, financial, and environmental healthcare needs.
- Educate patients and ancillary personnel about healthcare needs, safety issues, and benefits.
- Utilize electronic tools, such as EMR, to document assessments, clinical findings, plans of treatment, and progress toward goals.
- Observe, record, and report patient condition and reaction to drugs and treatments to physicians.
- Maintain and review patient records, charts, and other pertinent information.
- Respond to and refer incoming patient treatment-related phone calls.
- Maintain patient confidentiality and adhere to practice standards.
- High School Graduate
- Bachelor Degree in Nursing (BSN) required within 5 years for existing employees in the role
- 2 or more years of RN experience
- Consideration may be given to a TUKHS Ambulatory Clinic LPN with 4 years of TUKHS experience in lieu of RN experience
- Basic Life Support Heartsaver (BLS HS) certification from the American Heart Association (AHA) or obtained within 14 days
- Licensed Registered Nurse (LRN) in good standing in the state in which you will be working and proof of compact licensure submitted/applied for in the state in which you live
