Job Title: Registered Nurse Home Health

We are seeking a skilled Registered Nurse to join our team at Maxim Healthcare Services in Hampton, VA.

About the Role:

The successful candidate will be responsible for providing high-quality patient care in a home health setting. This will involve assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care delivery.

Responsibilities:
  • Assess patient needs and develop individualized care plans
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Collaborate with other healthcare professionals to ensure comprehensive care delivery
  • Document patient progress and communicate with healthcare teams
  • Participate in ongoing education and training to stay current with best practices
Requirements:
  • Current Registered Nurse (RN) license in the state of Virginia
  • One year of prior RN experience, preferably in home health care
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
