We are seeking a dedicated and compassionate Licensed Vocational Nurse or Licensed Practical Nurse (LVN or LPN) to join our team at Transition Home Healthcare. As a home health nurse, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.
Responsibilities:- Collaborate with our interdisciplinary team to develop and implement individualized care plans.
- Complete home care nursing visits, providing personalized attention and care to our patients.
- Prepare accurate and timely clinical notes, ensuring seamless communication with healthcare providers.
- Educate patients and their families on health conditions, treatment plans, and self-care techniques.
- Adhere to all agency policies, procedures, and regulatory requirements related to home health care.
- Valid and current LVN or LPN license in the state of Texas or compact.
- At least one year of experience in a home health setting.
- Knowledge of home health regulations and general healthcare compliance.
- Experience with HCHB EMR (preferred but not required).
- Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
