As a Home Health Physical Therapy Assistant, you will work closely with our team of healthcare professionals to develop and implement personalized treatment plans that promote optimal patient outcomes.
Responsibilities- Perform treatment and provide exercise instruction and patient education aimed at improving/enhancing the patient's well-being.
- Follow all state-specific laws governing the provision of physical therapy in home care.
- Collaborate with the supervising Physical Therapist to develop and implement treatment plans.
- Provide patient education and training on safe and effective use of prosthetic devices.
- Document patient progress and communicate with the healthcare team as needed.
- Current licensed, registered, or certified to assist in the practice of physical therapy in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
