About the Role

We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at Allay Home and Hospice. As a Hospice Care Specialist, you will play a vital role in providing personalized care to patients in their homes, supporting their physical, emotional, and spiritual needs.

Responsibilities
  • Provide personal care services, including bathing, dressing, and grooming, as assigned
  • Assist with daily living activities, such as meal preparation and feeding, as needed
  • Monitor and report changes in patient condition to the Case Manager
  • Participate in quality assessment and performance improvement activities
  • Collaborate with the hospice team to ensure comprehensive care and support
Requirements
  • Meet the training requirements of the State
  • Completed a minimum of 75 hours of classroom and supervised practical training
  • Minimum of 6 months experience in providing personal care
  • Ability to read and follow written instructions and document care given
  • Understands hospice philosophy and is comfortable providing specialized care to the terminally ill
