About the Role

We are seeking a skilled and compassionate Occupational Therapist to join our team at LHC Group, a leading provider of post-acute care services. As a Home Health Occupational Therapist, you will play a vital role in helping patients regain their independence and improve their quality of life.

Key Responsibilities
  • Develop and implement individualized therapy plans to meet the unique needs of each patient.
  • Evaluate patient progress and adjust treatment plans as needed to ensure optimal outcomes.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and training to patients and their families on proper techniques and strategies for maintaining independence.
  • Conduct thorough assessments of patient functional status and occupational therapy needs.
  • Consult with physicians to develop and implement therapy plans that align with patient goals and needs.
  • Document patient progress and outcomes in accordance with regulatory requirements and company policies.
Requirements
  • Current Occupational Therapy licensure in the state of Illinois.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
