Overview

As a Clinical Nurse specializing in Ambulatory Care, you will play a crucial role in delivering high-quality healthcare services. This position is located within a dynamic healthcare environment, where you will be part of a multidisciplinary team dedicated to providing comprehensive care to patients throughout their life span.

The Patient-Centered Medical Home (PCMH) model emphasizes a collaborative approach to healthcare, ensuring that all patient needs are met through a coordinated team effort. You will work alongside Physicians, Nurse Practitioners, Physician Assistants, and other healthcare professionals to deliver patient-centered care that includes disease prevention, wellness, and health management.

Key Responsibilities

1. **Deliver Comprehensive Nursing Care**: Provide complex nursing services to patients using evidence-based practices. Your responsibilities will include:

  • Conducting assessments and delivering care via telemedicine and in-person visits.
  • Performing invasive procedures and managing medication administration.
  • Tracking patient progress and ensuring continuity of care.

2. **Collaborate with Healthcare Teams**: Actively engage with clinic teams and other departments to enhance patient outcomes. This includes:

  • Participating in team meetings and care coordination efforts.
  • Advocating for patients and facilitating communication among healthcare providers.
  • Ensuring compliance with healthcare regulations and standards.

3. **Identify and Address Clinical Challenges**: Utilize problem-solving skills to enhance patient care delivery. Responsibilities include:

  • Mentoring and training team members to improve clinical competencies.
  • Leading quality improvement initiatives within the clinic.
  • Responding effectively to emergencies and ensuring patient safety.

4. **Additional Duties**: You may be assigned various responsibilities based on operational needs, contributing to the overall effectiveness of the healthcare team.

Qualifications

To be considered for this role, candidates must:

  • Be a United States Citizen.
  • Hold a valid Registered Nurse License.
  • Possess an accredited degree in Nursing (Associate's, Bachelor's, or Master's).
