Hospice Social Worker Role

We are seeking a compassionate and skilled Hospice Social Worker to join our team at Compassus. As a key member of our interdisciplinary team, you will provide essential support and care to patients and their families in need of hospice services.

The ideal candidate will possess a Bachelor's degree in Social Work, Psychology, or a related field, with one year of experience in a healthcare setting. A Master's degree in Social Work is highly preferred. Additionally, licensure as a social worker may be required depending on the state.

In this role, you will be responsible for providing preventive, educational, and evaluative services to meet the psychosocial needs of patients and their families. You will also participate in care planning meetings, facilitate placement in long-term care facilities, and support advance planning, including living wills and POA documents.

This position requires strong written and verbal communication skills, as well as the ability to work effectively in a team environment. If you are passionate about delivering high-quality patient-centered care and have excellent interpersonal skills, we encourage you to apply.

Salary Range: $60,000 - $80,000 per year, depending on experience and location.
