Position Overview:
Reports To: Nurse Manager
Purpose: Support the medical team in both inpatient and outpatient settings.
Compensation: Competitive hourly rate

Key Responsibilities:
  • Assist healthcare providers in patient preparation, including taking vital signs and managing prescription refills.
  • Request and manage medical records efficiently.
  • Facilitate care coordination for both inpatient and outpatient services.
  • Review and manage immunization records.
  • Act as a liaison with client physicians, coordinating appointments and medication orders.
  • Collaborate with the clinical team to provide insights on medical issues for potential clients.
  • Secure referrals and resources for participants and actively participate in team meetings.
  • Review health-related policies to ensure compliance with relevant regulations and recommend necessary updates.
  • Oversee procedures related to infection control and manage medical waste disposal.
  • Maintain accurate client information in electronic systems.
  • Ensure first aid kits and standing order medications are adequately stocked.
  • Order medical supplies as required.
  • Provide additional services as directed by the Medical Director or Nurse Manager.
Qualifications:
  • Current licensure as a Licensed Practical Nurse in Minnesota.
  • CPR certification for Healthcare Workers is mandatory.
  • A minimum of three years of direct service experience with diverse and underserved populations.
  • Experience in mental health or public health settings is preferred.
  • Strong attention to detail, excellent communication skills, and proficiency with technology.
  • Ability to pass a background study and meet applicable regulatory requirements.
  • Valid Minnesota Driver's License with a clean driving record.
  • Must be at least 18 years old and capable of lifting 25 lbs.
  • Ability to navigate stairs as part of job responsibilities.
