Salary Range

$65,000 - $85,000 per year (based on experience and location)

Job Description

We're seeking a skilled Registered Nurse to join our clinical team as a Nurse Care Specialist. As a key member of our interdisciplinary care team, you'll evaluate patients and develop personalized care plans that address their unique needs.

  • Evaluate patients and create comprehensive care plans
  • Collaborate with the care team to ensure seamless coordination of patient care
  • Communicate effectively with patients, families, and healthcare providers
Required Qualifications

To be successful in this role, you'll need:

  • A current state license as a Registered Nurse
  • At least 2 years of experience in a clinical care setting (hospice experience preferred)
  • Reliable transportation and ability to lift at least 25 pounds
