Job Summary
We are seeking a highly skilled and compassionate Nurse Practitioner or Physician Assistant to join our team in Arizona. As a Primary Care Specialist, you will play a critical role in delivering high-quality patient care and contributing to the growth and success of our organization.
Key Responsibilities
- Provide comprehensive primary care to patients in an outpatient setting, including routine check-ups, health screenings, and disease management.
- Partner with our care team to develop and implement personalized care plans that address the unique needs and goals of each patient.
- Utilize our proprietary Electronic Health Record system to track patient progress, identify areas for improvement, and inform care decisions.
- Collaborate with our medical director and other healthcare professionals to stay up-to-date on the latest medical research, best practices, and treatment options.
- Contribute to the development and implementation of new care models and initiatives that improve patient outcomes and enhance the overall healthcare experience.
Requirements
- At least 2 years of primary care experience required.
- Currently licensed or ability to obtain licensure in the state of Arizona.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare teams.
- Ability to work effectively in a fast-paced, dynamic environment and adapt to changing priorities and circumstances.
- Board Certification and Spanish language fluency a plus.
