Key Responsibilities:
  • Provide direct patient care and lead a team of medical office staff
  • Develop and implement case management plans to maximize health care responses, quality, and cost-effective outcomes
  • Communicate and coordinate with interdisciplinary teams, payers, and community resources to minimize fragmentation of services and maximize outcomes
  • Plan and conduct patient-centered team meetings to coordinate healthcare team members and third-party payers to achieve treatment outcomes and discharge planning goals
  • Provide education to interdisciplinary teams, patients, and families about treatment options, community resources, insurance benefits, and psychosocial concerns to ensure timely and informed decisions
  • Identify and allocate resources to improve quality of care and maintain a balance with cost-effective care on a case-by-case basis
  • Develop and implement discharge plans in collaboration with healthcare teams and families to meet patient needs and financial requirements
Requirements:
  • Bachelor's Degree in Nursing or related field
  • Current, unrestricted license to practice in Massachusetts as a Registered Nurse
  • Case management experience in an acute, rehabilitation, skilled nursing, or outpatient setting
  • Knowledge of community services and healthcare systems
  • Computer proficiency with Microsoft Office applications
