We are seeking a skilled Physical Therapist to join our team at Mount Carmel Health System, a part of Trinity Health, one of the nation's largest not-for-profit Catholic healthcare systems.
Job SummaryThe Physical Therapist will be responsible for evaluating and treating patients referred for physical therapy by a physician, communicating and collaborating with other caregivers to provide quality care, and maintaining accurate patient records.
Key Responsibilities- Perform physical therapy evaluation, establish and implement treatment plans for patients referred by a physician and assigned by the clinical coordinator or manager.
- Document accurately patient evaluation, treatment, progress, and discharge records.
- Delegate appropriate treatment procedures to the Physical Therapist Assistant, review, and co-sign their documentation.
- Communicate with appropriate health care providers on matters concerning patient treatment, continuity of care, and discharge planning.
- Maintain department readiness, i.e., supplies, linen, and equipment cleaning.
- Minimum Education Requirement - Graduate of an accredited physical therapy program.
- Licensure Requirement - Current license to practice physical therapy (PT) in the State of Ohio; Maintains yearly FIM certification (Inpatient Rehab Unit only).
- Physically able to perform strenuous activities, such as lifting and transferring patients.
- Must hold a valid Ohio driver's license.
