Position Overview

Provides clinical leadership for all facets of patient care in an outpatient environment under the supervision of the physician, Advanced Practice Provider, or physician resident.

Exhibits clinical proficiency by applying evidence-based practices and research throughout the nursing process:
assessment, planning, implementation, evaluation, and outcomes.

Integrates knowledge of safety protocols, cost efficiency, and quality enhancement throughout the nursing process.

Complies with all regulations established by The Joint Commission, Board of Nursing,

Board of Pharmacy, DHEC, CAP, COLA, and other regulatory bodies.

Key Responsibilities

Assessment:
Gathers data and conducts nursing assessments using appropriate area and/or age-specific standards and tools.

Identifies and responds to subtle or significant changes in patients' conditions and their responses to interventions. Manages patient phone inquiries and addresses clinical needs within the scope of practice. Employs critical thinking skills to direct patient care.

Planning/Outcomes:
Collaborates with Physician/APP/physician resident to formulate a care plan for patients.

Refers patients appropriately based on assessments (e.g., skin, nutrition, social services). - 15%

Implementation:
Delivers direct care to patients in a safe, timely, appropriate, and cost-effective manner, aligned with the established care plan and identified patient priorities.


Executes specific tasks based on organizational, departmental, and/or policy/procedure guidelines within the nursing scope of practice; utilizing known resources and appropriate technology both internal and external to the practice (as per evidence-based practice).

Delegates suitable nursing tasks to healthcare team members, ensuring completion of assigned responsibilities.

Monitors the quality of care provided.
Assumes accountability for patient safety by maintaining a secure environment, including awareness of safety procedures, protocols, and adherence to patient care standards (infection control, medication administration, etc.).

Evaluation:
Informs the provider of changes in patient conditions based on assessment/reassessment of patient responses to interventions. Documents appropriately in the medical record.

Patient/Family Education:

Identifies and assesses patient/family learning needs and educates accordingly by:

  • using language and terminology familiar to the patient/family
- providing examples to clarify concepts
- encouraging patient/family involvement in the development and evaluation of the teaching plan
- utilizing appropriate resources (e.g., written materials, digital aids, etc.)

Communicates the educational needs of the patient/family to others in the healthcare continuum, utilizing resources within the interdisciplinary healthcare team as necessary.

Professional Accountability:


Evaluates personal nursing practice in relation to professional standards, state regulations, policies, procedures, and current knowledge.

Takes personal responsibility for maintaining competency through education and professional development to enhance community healthcare.

Maintains professional licensure and completes required continuing education in a timely manner.

Reflects on identified strengths and areas for improvement.
Ensures that written documentation and oral communication focus on relevant information to support patient/family care needs.

Acts as the patient's advocate and care coordinator, collaborating with physicians and other healthcare team members.

Minimum Qualifications
Associate's Degree in Nursing. BSN preferred.
No prior experience required.

Licensure/Registrations/Certifications
Licensed to practice as a Registered Nurse in South Carolina.
