Job Title: LPN Home Health Nurse

Job Summary:

UPMC Home Health is seeking a full-time Licensed Practical Nurse (LPN) to join our team in Cambria County. As an LPN, you will work Monday to Friday daylight shifts from 8:00 a.m. to 4:30 p.m. with rotating holidays and occasional weekends if needed.

Responsibilities:

  • Provide direct, one-on-one patient care and establish rapport with patients.
  • Act as a member of the healthcare team by exercising sound nursing judgment based on preparation, knowledge, experience in nursing, and competency.
  • Communicate with a licensed professional nurse and the patient's healthcare team members to seek guidance when necessary.
  • Obtain instruction and supervision if implementing new or unfamiliar nursing practices or procedures.
  • Follow the written, established policies and procedures of UPMC, UPMC LPN Practice Guidelines, and the State Practice Act.

Requirements:

  • Successful graduation from a Licensed Practical Nursing Program approved by the State Board of Nursing.
  • Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.
  • Ability to work productively and effectively within a complex environment, handle multiple/changing priorities, and specialized equipment.
  • Physical stamina for frequent walking, standing, lifting, and positioning of patients.
  • Computer skills required to utilize the electronic health record.

Licensure, Certifications, and Clearances:

  • LPN must maintain state licensure where they are practicing or in a state covered by a licensure compact agreement with the state where the facility is located.
  • Basic Life Support OR Cardio Pulmonary Resuscitation (CPR) certification.
  • Act 33 and Act 34 clearances.
