Job Summary

Relies on education and training to complete responsibilities within scope of licensure as registered nurse.

Key Responsibilities
  1. Patient Care
    1. Conduct patient psychosocial/physiologic assessments that integrate changing data at all times.
    2. Formulate a plan of care based on trends of similar patients, including patient education and discharge planning, and tailor and prioritize caring practice to individual needs, including cultural/ethical/spiritual needs at all times.
    3. Participate in the planning of routine transitional health care needs, including treatment options, patient placement options, and end of life options, at all times.
    4. Respond to changes in patient's needs immediately and provide holistic care using independent clinical judgment based on knowledge drawn from education and experience at all times.
    5. Adapt planned educational programs and information to individual patients and family by modifying teaching strategies or content and integrate this education during the delivery of patient care, as evidenced by collaborating with patients/families to identify realistic desired outcomes.
    6. Ensure efficiency in the delivery of care and determine immediate priorities and offer solutions for problems as needed.
    7. Advocate for patient rights and identify potential conflict and seek counsel for resolution of conflict as necessary.
    8. Identify variances from expected outcomes and revise the plan of care based on patient outcomes, as necessary.
    9. Accurately document all patient care activities per documentation standards and provide assistance to the healthcare team with the documentation process as needed.
  2. Coordination of Care
    1. Provide consistent communication/collaboration with the health care team members, patients, and families to maximize patient outcomes at all times.
    2. Delegate patient care activities appropriately to other health care team members and participate in making and revising staff assignments to achieve maximum productivity as needed.
    3. Maintain knowledge regarding unit initiatives, as evidenced by the incorporation of the outcomes of the team/committees work into practice.
    4. Participate on unit teams, work groups, and/or committees as requested and anticipate needs and voluntarily assist others.
  3. Quality
    1. Serve as a clinical resource to health care team members, display leadership behaviors, provide direction and guidance to others regarding practice as needed, and act as a mentor and preceptor.
    2. Participate in quality improvement initiatives by utilizing standards, guidelines, and pathways for care delivery.
    3. Incorporate evidence-based practice and research findings into nursing practice to enhance outcomes at all times.
  4. Other Duties as Assigned
    1. Complete other duties as assigned.
    2. Participate on committees as needed.
    3. Act as a backup for others in the department as needed.
Qualifications

Graduate of accredited school of nursing

Proficient in the English language, verbal and written communication skills, computer skills

Current Louisiana State license as RN; BLS Certification; ACLS and PALS must be obtained within 6 months of hire date
