Speech Language Pathologist

Adoration Home Health and Hospice is seeking a passionate and dedicated Home Health Speech Language Pathologist to join our team. As a Speech Language Pathologist, you will have the opportunity to provide essential one-on-one care in the patients home while supporting their families. Responsibilities:
Perform evaluations and treatments of patients with speech, language, cognitive, and swallowing disorders based on physician orders/referrals.
Complete comprehensive evaluations using appropriate evaluation and assessment tools and procedures.
Review patient records for current diagnosis, past medical history, precautions, and contraindications.
Establish measurable goals and develop treatment plans to move patients toward their maximum level of speech, language, cognitive, and swallowing abilities.
Regularly re-assess the effectiveness of treatment plans, progress of patients toward goals, and the need for modifying goals or treatment.
Communicate patient needs and progress to the treatment team, physician, person receiving services, family members, as applicable.
Consult with medical providers regarding patient evaluation and treatment, progress of patients, and discharge from treatment.
Maintain and submit evaluation, treatment, and billing documentation in the format and time-lines required by the Company, facility, funding source, local, state, and federal regulations.
Qualifications:
Master's Degree in Speech Language Pathology.
Current Speech Language Pathology licensure in the State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire.
One (1) year of experience in speech pathology, home health experience preferred.
Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills; speech language pathology skills per competency checklist.
