We are seeking a highly motivated and experienced Pediatric Pharmacist to join our team at UNC Health. As a Pediatric Pharmacist - Clinical Specialist, you will play a critical role in providing comprehensive pharmaceutical care to patients in our Level IV Neonatal Critical Care Center (NCCC) and other pediatric patient populations.
Key Responsibilities- Ensure timely medication access for patients, including order review and verification, and clarify incomplete, inaccurate, or contraindicated prescriptions with prescribers, nurses, or other healthcare professionals.
- Utilize and analyze patient data to provide optimal drug therapy recommendations, assess medication therapy regimens, and make appropriate clinical interventions with providers.
- Perform protocol-driven clinical services, such as IV to oral dosing, stewardship activities, therapeutic dose monitoring, and antimicrobial stewardship.
- Compound and oversee the preparation and dispensing of medications, including sterile products, using standard formulas, processes, and procedures.
- Maintain patient profiles within the electronic medical record to facilitate communication.
- Comply with the medication formulary, therapeutic substitution, and other programs designed to optimize drug therapy while minimizing cost.
- Facilitate discharge and transfer of patients, including medication reconciliation and discharge counseling.
- Practice medication safety, including adverse drug event and medication error reporting.
- Participate in Code Blue situations for pediatric patients.
- Precept students, residents, and participate in training new pharmacy staff.
- PharmD or BS Pharmacy Degree
- Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment
- Completion of a PGY1 or PGY2 residency in pediatrics or an equivalent level of pharmacy experience
- Excellent initiative, communication, analytical skills, organization, and collaboration
This is a full-time position with a standard work schedule of 40 hours per week. The work assignment type is onsite, and the location is UNC Medical Center in Chapel Hill, NC.
