HOUSE CALLS NURSE PRACTITIONER - FNP/AGNP - APRN

At Abode Care Partners, we are seeking a compassionate Nurse Practitioner (NP) to join our team in delivering personalized medical care to seniors and patients with complex needs in their homes or assisted living facilities.

We offer a competitive compensation package ranging from $100,000 to $120,000 per year, based on performance. You will have the opportunity to grow your medical skills while providing quality care to those who need it most.

  • Perform evidence-based medical evaluations, treating chronic and urgent conditions, and collaborating with other healthcare professionals to maintain comprehensive care plans.
  • Examine and treat episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • Order and interpret laboratory and diagnostic testing as needed.
  • Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
Requirements:
  • A Nurse Practitioner National Board Certification.
  • An active state Nurse Practitioners license.
  • A current State DEA licensure.
  • A Master's degree in Nursing (MSN).
  • At least 5 years of nursing experience; at least 2 years of nurse practitioner experience preferred.
  • Strong interpersonal communication skills – verbal and written.
  • Excellent time management skills with accountability for practitioner's daily workflow.
