Nurse Practitioner

We are seeking a skilled Registered Nurse to join our Mother/Baby Unit at Northwell Health. As a key member of our team, you will be responsible for delivering high-quality patient care, collaborating with interdisciplinary teams, and contributing to a positive and supportive work environment.
Key Responsibilities- Assess and diagnose patient conditions, develop and implement individualized plans of care, and evaluate patient responses to care.
- Participate in patient education and discharge planning, ensuring patients and families are empowered to make informed decisions about their care.
- Collaborate with multidisciplinary teams to coordinate patient care, achieve goals, and promote positive outcomes.
- Develop and evaluate plans of care, systematically assessing patient knowledge, physical, psychosocial, and spiritual health through interviews, physical examinations, and data collection.
- Communicate effectively with patients, families, and healthcare teams, ensuring timely and accurate exchange of information.
- Provide and/or delegate patient care according to established protocols, utilizing clinical resources and rationale for patient care decisions.
- Conduct clinical rounds, provide emergency care, and administer medications as needed.
- Operate equipment required for patient care, adhering to manufacturer's recommendations.
- Perform and assist with procedures and treatments, including changing dressings, feeding patients, and providing pre/post-operative care.
- Practice and monitor infection control standards, promoting patient safety in accordance with state and federal guidelines.
- Document patient care, entering date and time of entry and signing name in a legible manner on patient charts.
- Complete Nursing Admission Assessments, Transfer Assessments, and Perioperative Assessments as required.
- Participate in Quality Improvement processes, developing and implementing unit goals and objectives.
- Attend in-service programs, complete occurrence reports, and participate in unit-based audits as assigned.
- Graduate from an accredited School of Nursing, with a Bachelor's Degree in Nursing preferred.
- Current license to practice as a Registered Professional Nurse in New York State.
- BCLS, ACLS, or PALS certification required.
