About the Role

We are seeking a skilled and compassionate Home Health Nurse to join our team at CenterWell Primary Care. As a key member of our care team, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Responsibilities
  • Provide skilled nursing visits to patients in their homes, including admission, case management, and follow-up care.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
  • Confer with physicians and other healthcare professionals to ensure comprehensive and coordinated care.
  • Monitor patient progress, identify areas for improvement, and make adjustments to care plans as needed.
  • Collaborate with other healthcare disciplines, including physical therapy, occupational therapy, and home health aides, to ensure seamless care delivery.
  • Document patient care and progress in accordance with regulatory requirements and company policies.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum of one year of nursing experience, preferably in home health or a related setting.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
