Lifespace Communities is seeking a skilled Nurse Care Specialist to join our clinical team. As a key member of our healthcare team, you will be responsible for delivering high-quality nursing care to our residents.
Key Responsibilities:
- Provide direct nursing services to residents on assigned neighborhoods or programs.
- Train and mentor new team members, ensuring they have the necessary skills to provide exceptional care.
- Oversee physician orders, verify accuracy, and administer medications and treatments as prescribed.
- Lead and supervise clinical and non-clinical team members, promoting a culture of excellence and teamwork.
- Collaborate with the interdisciplinary team to develop and implement resident care plans, ensuring seamless coordination and communication.
- Document resident conditions, nursing needs, and treatment outcomes accurately and in a timely manner.
- Coordinate admissions, discharges, and transfers, providing exceptional customer service and ensuring a smooth transition for residents.
- Plan and execute resident admissions, verifying orders and notifying relevant departments as necessary.
- Ensure continuity of care by providing thorough reports to on-coming shifts.
Requirements:
- Certifications and Registered Nurse license required, with additional state-specific licensure as necessary.
- One year of nursing experience in a long-term care facility preferred.
