We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Care Manager at Albuquerque Heights Healthcare and Rehab in Albuquerque, NM. As a Care Manager, you will play a vital role in providing high-quality patient care and leading our interdisciplinary team to achieve exceptional outcomes.
Key Responsibilities- Perform comprehensive nursing assessments and develop effective care plans in collaboration with the interdisciplinary team.
- Delegate patient care responsibilities to staff and supervise them to ensure high-quality care delivery.
- Communicate patient information with assigned staff and between shifts to ensure seamless care transitions.
- Establish realistic, measurable patient goals and interventions to minimize rehospitalizations and improve patient outcomes.
- Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- CPR Certification is required.
- One to three years of experience working as a RN in a related setting is preferred.
- A competitive hourly rate of $34 - $42 per hour.
- A dynamic and supportive work environment with a focus on patient-centered care.
