Your RoleThis full-time position involves conducting Face-to-Face (F2F) encounters, gathering clinical information to support hospice diagnosis and prognosis. You will report directly to our Executive Director or Administrator and work at an assigned location. Your findings will be communicated to our certified physician, ensuring timely patient care.
Key Responsibilities- Perform physical assessments for F2F visits within CMS-regulated time frames.
- Provide clinical findings to our certified physician, including review of medical treatment plans, diagnostic tests, lab results, and medication lists.
- Make recommendations to the Interdisciplinary Group (IDG) based on your assessment.
- Notify the hospice agency when unable to complete scheduled F2F visits.
We seek candidates with a Master's degree in Nursing and completion of a graduate-level nurse practitioner education program accredited by a national accrediting body. Current state licenses as a registered nurse and advanced practice nurse are required. Additionally, you must have a national certification as a Nurse Practitioner (ANCC or AANP) and a collaborative agreement in place.
