Job Summary:

As a Physical Therapist I PRN, you will play a vital role in the rehabilitation and wellness of our patients. Your expertise will be essential in evaluating and assessing patients to establish functional, measurable goals to promote restoration of functional abilities.

Responsibilities:
  • Evaluate and assess patients based on subjective and objective information.
  • Develop a written plan of care.
  • Provide treatment according to established goals on plan of care.
  • Document interventions/care in the patient's medical record.
  • Evaluate effectiveness of plan of care while also reassessing patient's functional status.
  • Provide discharge planning information which includes patient/family education, follow-up, and referral to community resources.
  • Contribute to the development of staff and students.
  • Participate in activities designed to improve health care delivery.
  • Complete assigned goals.
Requirements:
  • Minimum: Entry level.
  • Licensure, Registration, Certification: BLS Certification within 14 days of hire.
  • Special Skills: Basic skills such as keyboarding, using a mouse, and navigating sites.
