Job Summary

We are seeking a skilled Physical Therapist to join our team at Encompass Care Home Health. As a Physical Therapist, you will provide therapy services to patients in a home health environment, assessing patient needs and generating treatment programs with established goals.

Key Responsibilities
  • Evaluate patients to assess their needs and develop individualized treatment plans.
  • Plan and implement treatment programs, identifying short and long-term goals and methods to achieve them.
  • Supervise and direct licensed Physical Therapist Assistants in the implementation of patient treatment plans.
  • Review the quality and appropriateness of therapy services delivered, ensuring effectiveness and efficiency.
  • Communicate effectively with patients, families, and healthcare team members, providing education and support.
Requirements
  • Must meet all certification and licensure requirements as established by the state and national board associations.
  • Must maintain an active license in good standing throughout employment.
