Position Details

LOCATION: Family Medicine Practice

HOURS: Full-time, day shift with one evening weekly and rotating Saturdays

POSITION SUMMARY: Responsible for delivering nursing services to patients across various age groups, including pediatric, adolescent, adult, and geriatric populations.

KEY RESPONSIBILITIES: Qualified candidates will be expected to:
  • Offer clinical guidance via phone or electronic communication, adhering to established protocols and standards.
  • Provide tailored patient education on preventive care, chronic disease management, and medication use.
  • Prepare patients for appointments using established rooming protocols.
  • Administer prescribed injections and medications in compliance with departmental guidelines.
  • Assist healthcare providers with procedures and examinations as required.
  • Conduct Point of Care Testing for various health assessments.
  • Collect and prepare laboratory specimens for analysis.
  • Manage prescription refills as directed by the healthcare provider.
  • Support tracking of tests and overdue results.
  • Perform wound care, including suture removal and dressing changes.
  • Ensure exam rooms are well-stocked and sanitized after each patient visit.
  • Collaborate with providers to review and adjust care plans, monitoring patient progress.
  • Engage in proactive outreach for patients needing preventive and chronic care services.
  • Coordinate care among specialists and facilitate transitions of care.
  • Assist in managing referrals and connecting patients with community resources.
  • Participate in initiatives aimed at enhancing patient experience and care quality.
  • Engage in daily discussions with the healthcare provider.
  • Act as an advocate for patient experience, maintaining professionalism in all interactions.
  • Handle patient information confidentially in accordance with privacy policies.
ADDITIONAL RESPONSIBILITIES: Other duties may include:
  • Performing front office tasks as needed.
  • Occasional travel may be required; a valid driver's license is necessary.
  • Participating in on-call duties as required.
  • Completing other assigned tasks.
QUALIFICATIONS:

MINIMUM REQUIRED:
  • Completion of an accredited LPN or RN program.
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Pennsylvania.
  • AHA CPR/BLS certification within 30 days of hire.
  • Ability to travel to multiple locations as needed.
PREFERRED:
  • 1-2 years of nursing experience in a primary or specialty care setting.
  • Experience in triage, emergency care, or related nursing specialties.
