Home Care LPN Position Summary

At Trinity Health, we are seeking a compassionate and skilled LPN to join our home care team. As a Home Care LPN, you will provide one-to-one care to patients in their homes, utilizing cutting-edge technology and clinical knowledge to deliver exceptional patient outcomes.

Responsibilities:

  • Client assessment, care planning, intervention, and overall supervision of patient care and outcomes
  • Follow physician orders and act in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements, and our policies and procedures
  • Provide phlebotomy and wound care services as needed

Requirements:

  • Graduate of an accredited or state-approved school of practical nursing
  • Licensure as an LPN in the State of Georgia
  • A minimum one year of experience in an acute care setting; home care experience preferred
