We are seeking a skilled Occupational Therapist to join our team at LHC Group. As an Occupational Therapist in Home Health, you will play a vital role in helping patients regain their independence and improve their quality of life.

**Key Responsibilities**

  • Conduct comprehensive assessments of patients' functional status and occupational therapy needs.
  • Develop and implement individualized plans of care to address patients' goals and objectives.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and training to patients and their families on adaptive equipment and techniques to promote independence.
  • Evaluate patients' progress and make adjustments to plans of care as needed.

**Requirements**

  • Current Occupational Therapy licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
