Job Summary:
We are seeking a skilled and compassionate Registered Nurse to join our Home Care - Skilled Nursing Unit team at Novant Health. As a Registered Nurse, you will play a vital role in providing high-quality patient care and coordination of home health services.
Responsibilities:
- Conduct admissions, recerts, ROC, and discharges for the assigned case load.
- Conduct routine nursing visits as needed.
- Assess patient needs.
- Coordinate patient care between the patient, caregiver, physician, and other appropriate disciplines.
- Responsible for the completion of all home health documentation.
Requirements:
- Graduate of an accredited school of nursing required.
- Associate's or Bachelor's degree in nursing.
- Current valid North Carolina nursing license required.
- Basic Life Support (BLS) through American Red Cross or American Heart Association required.
- Driver's License Class C.
- Minimum 1 year of home health or related experience required.
