We are seeking a qualified In-House Speech/Language Pathologist to join our dedicated team.

Key Responsibilities:

  • The Speech Therapist will conduct assessments for patients referred for rehabilitation services, which includes providing direct care to evaluate their medical status, functional abilities, limitations, and rehabilitation potential.
  • They will design and implement a tailored treatment plan with specific objectives based on the patient's capabilities and tolerance, under the guidance of the Physician.
  • The Speech Therapist will establish realistic and attainable goals for patients, meticulously document their condition, and educate both patients and families regarding the care plan.

Qualifications and Requirements:

  • Graduation from an accredited Speech Therapy program or university.
  • Possession of a valid professional license and registration for the state of MN.
  • Experience in long-term care and/or geriatric care is preferred but not mandatory.
  • Proficient in EMR documentation, including evaluations, weekly progress notes, recertification, and discharge documentation.
  • Ability to collaborate with various disciplines to ensure optimal patient-centered treatment and care.
  • Exceptional skills in patient service and a commitment to demonstrating clinical excellence.
  • Adherence to scheduled hours and a commitment to professionalism at all times.
  • Fulfillment of additional duties as assigned by the Director of Rehabilitation, ensuring compliance with confidentiality and HIPAA regulations.
