Job Description

We are seeking a highly skilled Care Coordinator to join our team at the University of New Mexico Hospital. This is a full-time position working as a day shift, with an estimated salary range of $73,000 - $110,000 per year, depending on experience.

About the Role

This Care Coordinator role is crucial in ensuring the smooth transition of patients from hospital care to community-based services. You will work closely with healthcare teams to coordinate patient care plans, identify patient needs, and develop strategies to improve health outcomes.

Your key responsibilities will include:

  • Conducting timely discharge planning by anticipating patient needs and collaborating with physicians, nurses, and other healthcare professionals.
  • Developing comprehensive multidisciplinary plans of care effectively utilizing tools and resources.
  • Coordinating referrals for home health services, home infusion medications and supplies, oxygen, and equipment.
  • Maintaining computer-based tracking systems and compiling required reports and records.

You will also be responsible for facilitating and participating in conferences, promoting effective communication among healthcare team members, and providing routine verbal and written documentation for patient assessments and progress.

Requirements

To be successful in this role, you will need:

  • A Bachelor's Degree in Nursing or a related field (a Nationally Accredited Nursing Graduate is preferred).
  • At least 1 year of directly related experience in case management or a similar field.
  • A valid RN license in New Mexico or reciprocal agreement by NM.
  • CPR for Healthcare/BLS Provision or Professional Rescuer certification within 30 days.
