Physical Therapist

We are seeking a skilled and compassionate Physical Therapist to join our team at Life Care Center of Leominster. As a member of our interdisciplinary team, you will have the opportunity to make a meaningful impact on the lives of our patients and residents.

**Job Summary:**
The Physical Therapist provides direct rehabilitation care to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards. This includes assessing patient needs, developing and implementing individualized treatment plans, and educating patients and their families on optimal recovery techniques.

**Responsibilities:**
  • Assess patient needs and develop individualized treatment plans that address functional goals, mobility, and pain management.
  • Implement evidence-based interventions, including manual therapy, exercises, and modalities, to promote optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Educate patients and their families on proper use of assistive devices, wound care, and other relevant topics.
  • Collaborate with physicians, therapists, and other healthcare professionals to develop and implement comprehensive care plans.

**Requirements:**
* Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT) * Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment. * One year experience in post-acute care or related setting preferred * CPR certification upon hire or obtain during orientation
