Job Summary

We are seeking a dedicated Clinical Social Worker to join our team at Fresenius Medical Care. As a key member of our interdisciplinary team, you will play a vital role in supporting patients with End Stage Renal Disease (ESRD) and their families.

Responsibilities
  • Conduct comprehensive assessments of patients' psychosocial status, strengths, and areas of need to inform effective care planning.
  • Develop and maintain collaborative relationships with healthcare teams, patients, and family members to ensure seamless care transitions and optimal treatment outcomes.
  • Provide education and support to patients and families on managing kidney disease, adhering to treatment plans, and accessing available resources.
  • Collaborate with facility management, financial coordinators, and billing staff to resolve insurance-related issues and provide information on payment options.
  • Maintain up-to-date knowledge of local vocational/educational rehabilitation programs and assist patients in accessing these services.
Requirements
  • Bachelor's degree in Social Work or related field required; Master's degree preferred.
  • Minimum 1-2 years of experience in clinical social work, preferably in a dialysis setting.
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations.
  • Familiarity with electronic medical records and other software applications used in a healthcare setting.

At Fresenius Medical Care, we offer a competitive salary range of $65,000-$85,000 annually, depending on experience, plus a comprehensive benefits package, including health insurance, retirement savings plan, and paid time off.
