Salary InformationThe hourly rate for this position is estimated at $30.00-$32.50 per hour, providing a stable income and opportunities for professional growth.
About the RoleThis is a rewarding role that involves providing one-on-one patient care in the comfort of clients' homes. You will work closely with patients to assess their needs, develop personalized care plans, and deliver evidence-based interventions that promote health, quality of life, and peace of mind.
Your responsibilities will include:
- Conducting comprehensive assessments of clients' physical, emotional, and social needs
- Developing and implementing individualized care plans that address clients' unique requirements
- Providing hands-on care, including medication management, wound care, and mobility assistance
- Collaborating with other healthcare professionals to ensure seamless care coordination
- Maintaining accurate and up-to-date records of client interactions and care plans
To be successful in this role, you will need to possess the following skills and qualifications:
- A current, unrestricted license as an LPN/LVN in the state(s) of practice
- At least one year of experience as an LPN/LVN in an acute care setting or equivalent; new graduate nurses may also apply
- Current CPR certification and TB test
- Strong communication and interpersonal skills, with the ability to build rapport with clients and their families
- Ability to work independently and as part of a team, with flexibility to adapt to changing circumstances
