Job SummaryWe are seeking a skilled Physical Therapist to join our team of healthcare professionals. As a Physical Therapist with Pinnacle Home Care Inc., you will have the opportunity to work with patients in their own homes, providing personalized physical therapy services to help them recover from illness or injury.
Key Responsibilities- Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Develop and implement individualized treatment plans in consultation with physicians and other healthcare professionals.
- Provide physical therapy services to patients in their homes, including exercises, stretches, and other interventions to improve mobility and function.
- Teach patients and their families how to use adaptive devices, such as wheelchairs and prosthetic limbs.
- Record and report patient progress and any changes in their condition to physicians and other healthcare professionals.
- Participate in interdisciplinary team meetings and contribute to the development of patient care plans.
- Supervise Physical Therapist Assistants and Home Health Aides as needed.
- Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
- Licensed or registered by the State.
- At least one year of institutional experience.
- Home care experience and at least one year of supervisory experience (2 years preferred).
- One year of experience in an acute/sub-acute setting.
