Job Title: Wellness Nurse

The Phoenix at Hammond is seeking a dedicated and compassionate Wellness Nurse to join their team.

Job Summary:

The Wellness Nurse will be responsible for monitoring the health and wellbeing of our residents, providing leadership support and assistance with resident care and services. This role requires a strong commitment to delivering high-quality care and ensuring the overall well-being of our residents.

Key Responsibilities:

  • Monitor the health and wellbeing of residents, identifying on-going needs and services through the assessment and Personalized Service Plan (PSP) process.
  • Ensure proper follow-through and documentation for residents with a change in clinical care needs.
  • Assist the Wellness Director with completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident's monthly needs.
  • Inform the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out.
  • Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates.
  • Assist with direct resident care as needed.

Requirements:

  • Must be a Licensed Practical Nurse or Registered Nurse.
  • A minimum of one year of experience working with elders or disabled individuals, preferred.
  • Must have demonstrated leadership capabilities.
  • Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred.
  • Ability to work weekends, evenings, and flexible hours, available to our customers at peak service delivery days and times.
