Job Title: Licensed Practical Nurse (LPN)

Park Springs Communities is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As a LPN, you will play a vital role in providing high-quality nursing care to our residents, ensuring their physical, emotional, and social well-being.

Job Summary:

The LPN will be responsible for supervising the direct care of members, ensuring timely and appropriate administration of medications, and providing direct care as necessary. You will also be responsible for supervising Certified Medication Assistants and Certified Nursing Aides, providing inter-shift reports, and maintaining accurate documentation of all care provided.

Essential Duties:
  • Supervise direct care of members
  • Supervise Certified Medication Assistants and Certified Nursing Aides
  • Provide direct care as necessary
  • Provide and receive inter-shift reports
  • Maintain accurate documentation of all care provided
  • Transcribe orders as necessary
  • Review medications for availability, expiration, and proper documentation
  • Destroy and document expired medication
  • Work with members, family, and pharmacy to maintain adequate medication levels
  • Maintain self-respect, personal dignity, and physical safety of each member
  • Respond to emergency calls
  • Participate in family/member counseling
  • Complete Medication Administration Record (MAR) to Cart audit weekly
  • Complete chart check weekly
  • Complete month-end reconciliation
Requirements:
  • Current LPN licensure in the State of Georgia
  • At least 6 months of nursing experience with geriatric involvement
  • Passion for working with elderly people
Physical Requirements:

Frequent standing, walking, stooping, bending, stretching, squatting, and pushing. Must be able to lift, transfer, turn, and position members weighing up to 125 lbs. unassisted or with assistance of one for members weighing up to 125 lbs. Must be able to transport members via wheelchair, Geri chair, or shower chair, pushing up to 150 lbs. unassisted. Must be able to lift and carry up to 20 lbs frequently and 50 lbs occasionally.
