We are seeking a compassionate and skilled Medical Social Worker to join our team at Valle Verde, a HumanGood community. As a Medical Social Worker, you will play a vital role in ensuring the social service needs of our residents are met, and their care plans are developed and implemented.

Key Responsibilities:
  • Conduct social service assessments and develop care plans for residents
  • Facilitate group activities and support groups to meet the social and emotional needs of residents
  • Visit and interview residents to identify their needs and revise care plans as necessary
  • Oversee discharge planning for residents and coordinate with therapy, nursing, physicians, and family members
  • Monitor Medicare eligibility periods and provide required notices to residents and responsible parties
Requirements:
  • Masters of Social Work Degree
  • Licensed Clinical Social Worker designation
  • At least 3 years of experience working in a healthcare setting, preferably with geriatric residents
  • Managed care experience
  • Ability to interface effectively with residents, providers, management, and team members
