We are seeking a skilled and compassionate Hospice LPN to join our team at Baystate Health. As a key member of our healthcare team, you will provide high-quality nursing care to patients and their families, working closely with our interdisciplinary team to ensure comprehensive and compassionate care.
Responsibilities- Healthcare Team Relationships/Communication
- Schedule and perform patient visits, following proper nursing techniques and reporting patient problems to physicians, primary nurses, and supervisors as needed.
- Consult with Clinical Manager coordinators to plan patient care and schedule visits.
- Document patient visits in charts and order patient supplies as necessary.
- Make assessments and care plans based on age-specific needs, taking into account physiological, developmental, and psychosocial needs of patients.
- Development and Education
- Attend education and training programs for ongoing professional development.
- Additional Duties
- Perform additional duties as requested by supervisors.
- Adhere to system and department compliance policies, and any applicable laws and regulations.
- Attend and participate on Agency Committees as appropriate.
- Education and Certifications
- MA LPN license and completion of all annual licensure requirements.
- Basic Life Support - American Heart Association certification.
- Licensed Practical Nurse - National Council of State Boards of Nursing certification.
- Skills and Competencies
- Excellent interpersonal, judgment, and communication skills.
- Ability to function as a team member and work independently.
- Ability to be flexible.
- Reliable transportation.
- Ability to strictly adhere to confidentiality requirements.
