**Job Summary**

We are seeking a licensed Physical Therapist to join our team to provide in-home therapy services to patients in the state of Maryland.

**Responsibilities**

  • Provide physical therapy services to patients in their homes, based on the Medicare Part B Outpatient Model as prescribed by their doctor.
  • Meet the requirements for licensure as a Physical Therapist in the state of Maryland.
  • Have at least one year of experience as a Physical Therapist, with community/home health experience preferred.
  • Commit to a minimum of 5 cases (10 hours per week).
  • Comply with accepted professional standards and practice.
  • Possess and maintain current Liability Insurance.
