About the Position

We are seeking a skilled Case Management RN to join our team at TravelNurseSource. As a Case Management RN, you will be responsible for developing, implementing, and evaluating individual patient care plans.

You will work closely with patients and their families to manage their care and provide support. Your role will involve serving as a liaison between patients, their families, and healthcare providers.

Key Responsibilities:

Requirements:

• 1-2 years of recent experience in case management or a related field
• Active license or accreditation in one of the 50 US states
• Additional requirements and certifications as may be requested by the facility
