Job Type: PRN positions available

Job Purpose: To provide high-quality Physical Therapy services to residents in accordance with established standards of practice and company procedures.

Key Responsibilities:

  • Provide physical therapy services to residents, adhering to company procedures and productivity standards.
  • Develop and implement individualized treatment plans, setting short and long-term goals and methods to achieve identified objectives.
  • Conduct timely screenings of residents to determine need for intervention/treatment.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and effective communication.
  • Document all interactions and interventions in the NetHealth EMR system, ensuring compliance with CMS expectations.

Requirements:

  • Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
  • Active Physical Therapy license in the state(s) of practice.
  • Ongoing continuing education and professional development to maintain current licensure and certification.
