We are seeking an experienced Hospice Registered Nurse to join our team at Della Infotech in Walla Walla, WA. As a Hospice RN Case Manager, you will be responsible for managing a caseload of 15 patients, conducting 4-5 visits per day, and completing charting on the same day.

Key Responsibilities:

  • Manage a caseload of 15 patients
  • Conduct 4-5 visits per day
  • Complete charting on the same day
  • Perform all service level nursing, including routine, GIP, continuous, and respite care
  • Perform admission, SIA, and death visits

Requirements:

  • 2 years of Hospice case management experience required
  • Acute/Med Surg experience preferred
  • BLS (AHA) certification required
  • Local RNs welcomed at reduced 10% off posted BR
  • MUST have active DL & auto INS

Profile Requirements:

  • 4 year work history, if applicable
  • Skills checklist within 1 year
  • '1' supervisory reference
  • Certificate(s)
  • Verified RN license

Certification Requirements:

  • BLS (AHA)

State License Requirements:

  • Washington

Additional License Requirements:

  • Oregon RN license is strongly encouraged

Weekend Requirements: As needed

On Call Requirements: As needed
