We are seeking a highly skilled Speech-Language Pathologist to join our team at EmpowerMe Wellness. As a Speech-Language Pathologist, you will be responsible for providing speech-language pathology and wellness services to older adults in a skilled nursing care facility (SNF) setting.

Responsibilities
  • Conduct comprehensive assessments and develop individualized treatment plans to address the communication and swallowing needs of residents.
  • Provide evidence-based therapeutic interventions to improve functional communication, cognitive-linguistic skills, and swallowing function.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care and coordination.
  • Maintain accurate and detailed records of patient progress and treatment outcomes.
  • Participate in quality improvement initiatives and contribute to the development of clinical policies and procedures.
Requirements
  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current licensure as a Speech-Language Pathologist in the state of practice.
  • ASHA certification in Speech-Language Pathology.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong analytical and problem-solving skills, with the ability to adapt to changing situations.
